Accessibility Booking Form

- Please be aware that applications can take up to 5 working days to process upon receipt of a completed application and supporting documentation.
- You must complete the form before making a booking. Bookings made before joining the scheme are not eligible for an essential companion ticket.
- Essential Companion / Carer tickets are not open or intended for those who simply want a companion or require assistance with transportation to our venues, and is limited based on the capacity of our venue and the event.
- The account must be in the name of the person with the accessibility requirement and not in any other name e.g. the name of the essential companion and essential companions must be aged 18+
- Entrance will not be permitted for holders of carers/ essential companion tickets unless they are accompanied by their named Access for All customer.
- Our team will endeavour to make reasonable adjustments to facilitate your request; however, due to capacity and space within our venues, it may not be possible to facilitate all requests.
- Please ensure you have completed this form before purchasing tickets as essential companion / carer tickets cannot be added to bookings retrospectively.
